Each Client seeking services out of network with their insurance will receive a detailed Good Faith Estimate for their review and signature as part of the onboarding process to become a client with Transitions Delaware.
Good Faith Estimate (GFA): Generic Statement for Educational Purposes: Good Faith Estimate
The No Surprises Act includes a requirement for health care providers to post a Good Faith Estimate on their practice website. This GFA is generic and educational because it is impossible to know diagnosis and estimated length of treatment even before meeting a person/couple/poly/family. This document is to help future clients have a better understanding of what to expect when they initiate services through Transitions Delaware LLC.
